An exciting opportunity has become available for a Paediatric Nurse to join our Children’s Community Nursing Service.
The base will be agreed with the successful candidate – Carmarthen/ Haverfordwest/ Cardigan.
This post is in partnership with WellChild whose mission is to make far-reaching improvements to children and young people with serious and complex health needs and this post joins a growing network of WellChild Nurses throughout the UK. The post holder will attend and participate in meetings and other activities as required by WellChild and act as an Ambassador for WellChild at all times.
The WellChild Nurse working within the Children’s Community Nursing Service will lead in the development & progression of nursing services for transitional care. A key component of this role will be to support the resilience and capacity development for young people, their families and services involved in their care.
As a member of the Children’s Community Nursing Team you will have continuing responsibility for a caseload ensuring high standards of care for children in their own homes or/and community setting. The role will involve educating, assessing, planning, implementing and evaluating care in partnership with the child and their family.
This role demonstrates the importance of multi disciplinary, multi agency working and ensures that the successful candidate has a clear understanding of the policies and procedures associated with supporting children and young people.
