The Kentown Children’s Palliative Care Programme is a community-focused children’s palliative care initiative in North West England covering Lancashire and South Cumbria.
It is a new groundbreaking and transformative project to reach the growing numbers of children and young people with life-limiting and life-threatening conditions.
Family Support Workers: Offers flexible practical and emotional support, helping families manage daily pressures and cope with the challenges of caring for a child with complex needs.
Kentown Nurses: Provide advances clinical care and acts as a consistent point of contact, delivering responsive support in the home and coordinating with wider services.
Service Coordinators: Connect families with the right services and resources, reducing barriers and making it easier to navigate complex health and social care systems.
The Kentown Children’s Palliative Care Programme is a collaborative programme between Together for Short Lives, The Kentown Wizard Foundation and Rainbow Trust Children’s Charity.
