Sunflower House
Sunflower House provides shared care and short stays for children with disabilities, sensory impairments and learning disabilities. They can also have complex health needs. Children can come to Sunflower House for a variety of reasons. For your child to make new friends, have fun and be given the opportunity to participate in new activities and gain more independence. It is also time for families and carers to have a break. Staff cater for a wide range of care, medical and dietary needs and are all fully trained in these areas. Everyone works together as a team to offer children individual choice and opportunities. The staffing levels vary depending on the children’s needs. Sunflower House is a purpose built bungalow with five bedrooms, located in a quiet residential area close to local amenities, bus stops and parks. There are spacious bedrooms with an intercom system, lounge / dining room, conservatory, kitchen, bathrooms, toilet facilities and laundry room. Admission to Sunflower House is through referral from the Children with Disabilities Social Work Team.

HCRG Care Group provide Wiltshire Children’s Community Services across the county. They provide a wide range of children and young people’s services which look after the physical, medical and mental health and wellbeing of children and young people up to the age of 19 years old. My Life Team – Kirklees Council
The My Life team attend Education, Health and Care plan (EHCP) reviews for young people from year 9 and post 16 provisions (14-25 years old). They work with young people and their families who do not have a social worker or a short breaks plan in place. They offer information, advice, and guidance on adult social care. They provide information about services that are available whereby you may not need an adult social care assessment and we will also offer information and advice about when and how to get an adult social care assessment if you do need one.

Open Access Play
Torfaen Play Service provides community play opportunities for children and young people aged 5-15 years in the form of after school clubs, half term and summer play schemes. In addition to this, support is provided for children and young people aged 5 - 18 years with disabilities to access community play provision. Support is provided to young people aged 16+ to volunteer within community play settings during holiday times, including young people with additional needs. Full training is provided. Term time training placements are available with Torfaen Play Service. This includes support to learn work-based skills, to work in a variety of settings including schools and community based venues as well as achieve an accredited qualification. Cardiff Disability Support With Leisure Service
This service supports children and young people with disabilities and additional needs to access leisure activities in the community to improve their confidence, integration and independence, whilst offering parents respite. Support with Leisure works in partnership with Cardiff local authority, who are the sole referrers. The Support with Leisure service works with children 0 – 18 and runs all year round, evenings and weekends.
