Middlesborough Early Years SEND Team
The Early Years SEND Team is an integral part of Early Help services and support young children with Special Educational Needs and Disabilities from birth to school age and their families. They help by providing support for the early identification of children with special needs, giving advice to registered settings on suitable play equipment, support with curriculum activities to promote inclusive practice, and supporting children attending Specialist provision. The Early Years SEND Service is available to families who: Live within the borough of Redcar and Cleveland Have a preschool child who has significant delay in at least two areas of development.

Me2Club
Me2 Club is an inclusion charity for children and young people with additional needs and disabilities in the Wokingham and Reading Boroughs. Their purpose is to tackle the social isolation and loneliness experienced by children not being able to access mainstream leisure activities. They do not require a formal diagnosis to offer support. They support children and young people aged 5-19 to take part in mainstream activities by recruiting and training volunteers to ‘buddy’ 1:1 with them (2:1 when required) so they can join in and have fun. Children can access a wide range of activities including sports clubs, groups, and drama classes, giving them a chance to build independence and vital life and social skills. Me2 Club also run 3 participation projects: • TeenW&Rd – Opportunity for young people aged 13-19years to make new friends and to build their independence by choosing, planning and fundraising for activities. • Include Me2 – Our Youth Social Action Group for ages 8 – 25 years and their siblings. Supporting young people to raise awareness of additional needs to address the barriers they face in their lives and in their communities. • Say YES – Wokingham’s SEND Youth Forum for 8 – 25 year olds. HCRG Care Group provide Wiltshire Children’s Community Services across the county. They provide a wide range of children and young people’s services which look after the physical, medical and mental health and wellbeing of children and young people up to the age of 19 years old. They provide everything from public health nursing (also known as health visiting and school nursing), to specialist support for children and young people with physical sensory or learning needs and disabilities.

My Life Team – Kirklees Council
The My Life team attend Education, Health and Care plan (EHCP) reviews for young people from year 9 and post 16 provisions (14-25 years old). They work with young people and their families who do not have a social worker or a short breaks plan in place. They offer information, advice, and guidance on adult social care. They provide information about services that are available whereby you may not need an adult social care assessment and we will also offer information and advice about when and how to get an adult social care assessment if you do need one.
